This is a very rare error we've seen sometimes when there is a system glitch when loading for the first time. The other reported time was with a power outage and a dat file became corrupt.

Solved,
I uninstalled the tool (was not in the fSX folder) , reboot, installed Accu Sim in the FSX folder and all OK
